HIV prevention has come a long way — but actually getting access to it in New York City can still feel complicated. Long waits at sexual health clinics, awkward conversations with unfamiliar providers, and insurance hurdles often stand between people and the medications that could protect them.
It doesn't have to be that way. Here's what you need to know about PrEP, Doxy-PEP, and HIV prevention in NYC — and how to access it without judgment or bureaucracy.
What Is PrEP?
PrEP (pre-exposure prophylaxis) is a medication that significantly reduces the risk of contracting HIV. When taken consistently, PrEP is highly effective at preventing HIV transmission through sex.
PrEP comes in a few forms:
Daily oral pills (such as Truvada or Descovy)
Apretude or Yeztugo — a long-acting injectable given every two or six months, ideal for people who don't want to remember a daily pill
Who Should Consider PrEP?
PrEP is recommended for anyone at ongoing risk of HIV exposure, including people who:
Have a partner who is HIV-positive
Have multiple sexual partners or partners of unknown HIV status
Don't consistently use condoms
Have had a bacterial STI in the past 6 months
Inject drugs and share equipment
There's no "right" reason to be on PrEP beyond wanting to protect your health. A good provider won't make you justify your choices — they'll simply help you access the medication safely.
What About Doxy-PEP?
Doxy-PEP (doxycycline post-exposure prophylaxis) is a newer prevention strategy where a single dose of the antibiotic doxycycline is taken after sex to reduce the risk of bacterial STIs like chlamydia, gonorrhea, and syphilis.
It's not a substitute for PrEP — the two address different things — but many patients use them together as part of a comprehensive sexual health plan.
What to Expect When Starting PrEP
Starting PrEP typically involves:
A baseline visit — discussing your health history and sexual health goals
Lab testing — HIV test, kidney function, STI screening, and (for some medications) Hepatitis B status
Prescription — choosing between daily oral PrEP or Apretude based on your preferences
Follow-up monitoring — typically every 3 months for oral PrEP, or aligned with injection visits for Apretude or Yeztugo
At U Care MD, all of this happens within your membership — no separate sexual health clinic, no waiting weeks for results, and no judgment.
Why See a Direct Primary Care Doctor for PrEP?
Many people default to sexual health clinics for PrEP because they assume that's the only option — but a Direct Primary Care doctor who's knowledgeable and affirming can be a far better fit:
Same doctor every time — Dr. Chen gets to know your full health picture, not just your sexual health
No long waits — same or next-day appointments when you need them
Privacy — your care isn't tied to insurance claims that might be visible to family members on shared plans
Integrated care — PrEP, mental health, primary care, and everything else, all in one place
Cost and Access
PrEP medications are often available at low or no cost through manufacturer assistance programs, regardless of insurance status. Dr. Chen can help you navigate these programs as part of your care — so cost doesn't have to be a barrier.
